• The “Wake Now Our Vision” (WNOV) form which informs the UUA of your intention to make a bequest and qualifies the Conejo Valley Unitarian Universalist Fellowship to potentially receive the 10% matching funds. Please note the size of your bequest is confidential and not revealed to CVUUF until your passing. It will only be used by the UUA to gauge the amount of matching funds to be provided in total to CVUUF.
• A form for our CVUUF files documenting your gift intentions.
• A list of resources for estate planning (which CVUUF does not endorse, but provides for your convenience).
• A copy of the CVUUF policy for “The Stability Fund” which will receive any money obtained outside the normal pledge and fundraising activities. This will include the WNOV matching funds and any legacy gifts received.  Available on request is a copy of the CVUUF Restricted Gifts policy.  If you are thinking about designating how your bequest will be used by CVUUF, you should be aware of the restrictions.
